Certificate of Financial Responsibility for Non-degree Students and Groups

 Please complete this form if you are coming to MIIS to take a non-degree program including:
  1. Custom Language Services (CLS)
  2. Special Groups
  3. Center for Nonproliferation Studies (CNS) Language Training
  4. Development Project Management Institute (DPMI)
  5. Global Trade & Development Program (GTD)
  6. Conservation Leadership Practicum
  7. Frontier Market Scouts (FMS)

The Office of Student Services provides immigration-related support to all international students. This online form may be completed by all non-degree and group students requesting an I-20 or DS-2019 for F-1 or J-1 student status. Because this is not a secure website, you have the option to submit a paper Certificate of Financial Reponsibility through regular mail/fax. Please contact Student Services for the paper application.

 Special note about visitor Visa Waiver, B-1 or B-2 status: full-time study is not permitted in the B-1 (business) or B-2 (visitor) visa or in the visa waiver program. The B visa does permit visitors to the U.S. to take short-term classes that are not the primary purpose of the visitor's presence in the U.S. If your program will be full-time, you will need to obtain an F-1 student visa. Why do we request evidence of financial support?

The U.S. government requires MIIS to obtain evidence that applicants have adequate funds to pay for their educational and living expenses while studying in the United States. Therefore, you may complete this form and submit it along with any required financial support documents (bank statements, letters of sponsorship, letters of financial aid, etc.). The tuition for custom non-degree programs and groups will vary according to the program.  If you are in a group, your financial support documents will be provided by your group coordinator.

 When will I receive my I-20 or DS-2019?

Your I-20 or DS-2019 will be processed once you have paid the tuition deposit and submitted evidence of financial support. In addition, the following documents are required :

  1. Bank statements or other financial support documents.
  2. Letter of support from family or private sponsor (with bank statement)
  3. Copy of your passport
  4. Copy of your dependent's passports, if applicable

Your documents will be sent by express mail once ithey have been created.

How do I complete this form?

This online form does not allow you to save information, so please take the time to look through the questions before you begin. You are able to attach documents, so you will want to have scanned your passport page and financial documents before completing this form. If you are in a group program and/or are sponsored, you may not have the financial verification forms.  Please don't worry because Student Services will get this directly from your coordinator.
