Online Certificate of Financial Responsibility (CFR) Form
Congratulations on your admission to the Monterey Institute of International Studies. The Office of Student Services provides immigration-related support to all international students. This online form may be completed by all international students requesting an I-20 or DS-2019 for F-1 or J-1 student status. Because this is not a secure website, you have the option to submit a paper Certificate of Financial Responsibility through regular mail.
Why do we request evidence of financial support?
The U.S. government requires MIIS to obtain evidence that applicants have adequate funds to pay for their educational and living expenses while studying in the United States. Therefore, you may complete this form and submit it along with any required financial support documents (bank statements, letters of sponsorship, letters of financial aid, etc.). When will I receive my I-20 or DS-2019?
Your I-20 or DS-2019 will be processed once you have paid the tuition deposit and submitted evidence of financial support. In addition, the following documents are required :
- Bank statements or other financial support documents.
- Letter of support from family or private sponsor (with bank statement)
- Copy of your passport
- Copy of your dependent's passports, if applicable
How do I complete this form?
This online form does not allow you to save information, so please take the time to look through the questions before you begin. You are able to attach documents, so you will want to have scanned your passport page and financial documents before completing this form. If you decide not to attach your documents, you may fax them (1+831-647-3570) or send them to the following address:

I am currently attending school in the US as an F-1 or J-1 student. What do I need to do?
If you are currently attending school as an F-1 or J-1 student, you should talk with your current international student advisor about a transfer of your SEVIS record. If there is less than five months between the end of your current program and the start of your program at MIIS, then you may stay in the United States on the same SEVIS record. If the time between programs is longer than 5 months, you should consult with your advisor or Student Services. If your current school requires a transfer form to release your record, please provide him/her with the MIIS Transfer Form. Please note, our office does not require this form as the release of your record is sufficient.
