Address Update

As a student on OPT, you are able to work anywhere in the U.S.  Please remember that you still need to update our office with your current local address within 10 days of moving.  If you have recently moved, please make sure to fill out the Address Update form. 

Please write your name as it appears on your I-20.

Please provide both the city and state in which you live.

Please provide the zip code of the city in which you live.

Please provide an email address, other than your MIIS email address. This email address will be used to send information information for students on OPT.

Please provide your SEVIS number. This number starts with an "N" and can be found above the bar code on your I-20 form.
