1. Severe Events Definition: If a fire alarm has been activated, if a fire has occurred, if injury or exposure to injury warrants emergency response, if a crime is in progress, or if property is imminently and significantly threatened, etc.   Since events that are severe may occur even though they are not listed above, the Reporting Officer may be required to exercise judgment on a case by case basis.

2. Notifications Hierarchy: If an event is severe, the Reporting Officer will always call 911 first before initiating other notifications to hierarchy.  If an event is not severe, the Reporting Officer will call the appropriate hierarchy personnel first before calling 911.

3. Incident Report: In all cases, the Reporting Person must document details of the event, and the Director of Security will create and submit an incident report upon completion of the investigation.

4. Email: Whenever a sensitive or delicate matter surfaces, avoid use of email conversation and make every attempt to immediately contact the appropriate personnel.

5. Confidentiality: Confidentiality should be maintained at all times and use the appropriate list of employees as designated to help resolve the issues. A breach of confidentiality may cause a disruption to an investigation, increase the liability risk to the Institute and potentially harm person(s) involved. At no time should any matters be shared with others unless prior authorization is granted.
