Event Submission Form

Planning a club event or activity? Submit an event using this submission form. 

Please complete this form if you are:
1) Requesting approval for a student club event or activity

and/or:

2) Requesting that your event be posted on the MIIS Events Calendar.

A staff member from the Student Affairs Office will contact you by email or phone to confirm whether or not your event has been approved and will be posted to the calendar.

Please list first and last name of main contact person here.

*  The name of the event should be listed how you want it to appear on the MIIS events calendar. 

 Panel, conference, brown bag lunch, film series, etc.

Outside the MIIS community

IMPORTANT:  Approval is needed to serve alcohol.  Alcohol cannot be served if the event is "open" to the public.

If you are planning on serving food/beverages, please provide a detailed description of what you plan to serve.

Once your event has been approved, it can be posted on the MIIS events calendar.  Please provide a short description of what you would like posted.  Remember to keep it short and to the point, this is your chance to make your event stand out!
