The following forms are commonly used by international students
I-20 Extension Form
if you will not complete your program by the current date on your I-20, use this form to request an extension.
Leave of absence
The following two forms provide information on filing a leave of absence with the Institute. In order to take a leave of absence please be sure to read the Leave of absence information before completing the Petition.
Program Completion Notification
This form should be completed by all F-1 & J-1 students upon completion of degree program. This form assists OIS in taking the proper action with your SEVIS record.
Underenrollment Forms for F-1 or J-1 Students
International students in F-1 or J-1 status may take less than 12 units in certain circumstances. If you are an international student and need to take less than 12 units due to a medical (physical or psychological) problem, please complete the following form. This must be submited with proper documentation from a licensed doctor or psychologist. You must receive this approval before dropping any classes. For more information, please schedule a meeting with an advisor at OIS.
Student Letter Request
This form can be used for students to request a letter for one of the following: Social Security Application support letter DMV Application Verification of student F-1 or J-1 status Visa support letter for student Visa support letter for visiting relative or friend.
